  I check with bug output decoder, your error is narrow to your ios: make sure you have a working, valid ios.
  Then make sure it supports for your specific 3640. Remove all NM cards to isolate HW issue.
  Swap to a known working flash / DRAM to further isolate HW but this is your last resource.
