Thank you for your responses. I will try adding "no exec" tonight when I
get home and I am sure that will resolve the issue. I configure the
Terminal Server according to below link and there isn't anything about "no
exec" command.

Reza,
        Use the "no exec" command under the lines (1-16). When your access
server receives messages from the console of the devices connected to it,
the access server starts an exec process. The "no exec" line command stops
this behavior.

        This is why you are seeing sessions that you did not initiate and in
turn having to clear the lines.

I have a random issue with my terminal server. I have a 2511 that I use with
an Octal cable to connect to my routers. Frequently I will see a line go hot
without me having initiated a session (I see an asterisk by the line in
'show lines'). If I clear the line using 'clear line x' and then open a
session to that device I will get about a hundred lines of trash thrown to
the end device as input. Once the trash scrolls by and errors out, I have
regular prompt access to the router with no further issues. There are also a
lots of overrun and noise on these lines. This is not an all the time
thing, but it is fairly frequent. Here's my setup on the terminal server:
ip host r1 2001 1.1.1.1
ip host r2 2002 1.1.1.1
ip host r3 2003 1.1.1.1
ip host r4 2004 1.1.1.1
ip host r5 2005 1.1.1.1
ip host r6 2006 1.1.1.1

flowcontrol hardware (just added this recently )
And here's the config for one of my end routers:
