Try upgrading to 121-11.EA1a
There is a documented bug in spanning tree in 121-EA9c

Subject: 3550 and spantree problem
Has anyone had problems when connecting the 3550 to another switch. such
as a 5000 or 1900, and configuring the 3550 as the root switch? When I
do this the spantree loop protection seems to fail.
If I take a 3550 and 1900 with default configs and connect them together
via 2 ports, the 1900 becomes the root and one port on the 3550 goes in
to blocking mode. If I change the root to the 3550 by reconfiguring
either switch, both ports go in to forwarding and stay there. I have
also tried this with a 3550 and 5000 with the same results.
The 3550 is running 121-EA9c. Any thoughts?
Thanks
