Are you looking for a trending- or polling application?
You might want to take a look at observium too; http://www.observium.org

We're using a mix of ez-cacti, nagios and observium. Nagios is still
the number one polling application (imho) with the large community and
scripts available.

Nothing seems to be the 'this can do it all'. At least that is my
experience with these kind of applications. :)
