I'm not offering this up as a solution to your problem but check out
Wacom's "Bamboo Paper" app in the App Store. It's free (for now?).

On first glance it looks like it could be used to sketch drawings and
then email them in PDF format to another person. Not real time or
collaborative but still potentially handy.
