What leads you to believe that Switch#1 and #4 are both acting as Master? Looking at the output from your show switch, it appears that Switch #4 is the only Master switch in the stack. Switches #1 & #2 appear to be only acting as members...
In regards to your provisioning issue, I have seen before where a switch in a stack was renumbered and the show switch output showed the "Member 0000.0000.0000 0 0 Provisioned" message like you are seeing. The problem was caused due a cabling issue. Be sure to check the stacking cables in the back to make sure that you have them cabled correctly. Switch 1 Stack Port 1 should be connected to Switch 2 Stack Port 2 and etc...
If that doesn't work, try doing a "no switch 2 provision" from the CLI.
