Some hits:
You've got 2xPVDM2-16 in your router.
1. Take both of them out physically and then reboot the router. (also take
any E1 or FXS/FXO module out of the box).

2. Power off the router - insert DSP#1 into slot#1, boot the router

The whole purpose of this excercise is to make sure:
a) your both PVDM's are okay
b) Your both slots are okay.
Once above is done then you can go and try your IOS upgrade.
