Some PVDMs are ... not that good, and they fail to start reliably.
You can power bounce the router to see if they both come up.
A better way is to reset them, with test reset:
Router# test dsp device <slot> <id> reset
<id> would be 1 for PVDM2-16, <slot> would be 1 or 2.
