If by "fully expired", you mean that you went through the two year recert
and the one year suspension period, then you have to start from scratch to
be an active R&S CCIE holder, and would need to take the written and lab.
If it has just been the two years for recert, and you are currently
'suspended', then passing a written can bring you back to active status, at
least for the next year.
One question would be why did you let it expire?
