I really have to lab it up today and will post my observations.
I thot it could be with same concept as we do with PLAR mode.

*With H323 gw its real easy to block, here are the steps*

voice translation-rule 100
 rule 1 reject /444444444/ <---- ANI of spammer
 rule 2 reject /5555555/

voice translation-profile block_spamcalls
 translate calling 100

Then apply it to the incoming dialpeer or global dialpeer:

dial-peer voice 100 pots
 call-block translation-profile incoming spamcalls
 direct-inward-dial
 incoming called-number .
 port 2/0/1:23

Yup, IPMA is the other way to do it.
