Hello,

It is simple, just open a case using the like below, plus you must have a user
name; if you do not have you can register to get one.
 http://ciscocert.custhelp.com

Before you ask them to provide you an invitation letter, you must have already
registered for the LAB.

Then you provide them the required information as below, in addition to the
LAB location.

Passport Information (all information as shown on passport):Surname:Given
Name: Passport number: Passport Issue date (dd-mmm-yyyy): Passport Expire date
(dd-mmm-yyyy): Place of issue: Nationality:Date of Birth (dd-mmm-yyyy):Travel
Information:Lab Exam Location:Name of person/company responsible for travel
expenses:Departure Date (dd-mmm-yyyy): Return Date (dd-mmm-yyyy):
Personal/Business Information:Nature of relationship with Cisco (example:
Temp, Vendor, Partner, or Client): Company Name:Company Mailing/Correspondence
Address (include street number, street name, apartment #,
city/province/region, country, zip code or postal code):Home
Mailing/Correspondence Address (include street number, street name, apartment
#, city/province/region, country, zip code or postal code):Company Phone
Number (include area code and country code, if applicable): Home Phone number
(include area code and country code, if applicable): Fax Number (if
applicable):
After that they will send it to you as PDF format,
Best regards,
