Well, it might just be that the MoH server assigned to the GW streams the audio file (that corresponds to the IP phone) to the IOS MTP which in turn streams it to the PSTN

Hi group,
When configuring an MTP on an IOS gateway and registering it with a
Call Manager (MTP registered through SCCP but Voice Gateway uses H323 to
communicate to the CCM), how comes when an IP phone (registered with the
Call Manager) invokes a supplementary feature (hold for instance) the
MTP software on the IOS gateway streams an audio file ( to the PSTN
behind the IOS gateway) that is located in the Call Manager server?
Any one knows how the MTP soft is reading the audio source? What
protocol is used etc...
