Hi,
This could be due to hardware failure. The network module you have installed
is a 2FE2W and it just shows only 1 FE. see below:

I had a similar problem with 1E2W card - router showed only a 1E, no matter
what WIC I install in this NM. I swapped this 1E2W card with another 3640,
and all was good except this particular card. So I suspect, there is a
problem(hardware) with your network module.
