Good document on Bc Be Tc

Hello Group,

Can anyone recommend a document or book that clearly explains (in human
terms) the meaning of Bc, Be, Tc.
I've read about them in frame relay books and more recently in QOS world but
my head starts spinning each time I try to understand them.
Also does anyone know the origin of these terms - they seem to be the
industry standard way of describing burstiness?
