As mentioned by Mike, this can occur if you have a route pattern or
translation patter that has the "provide outside dialtone" box unchecked or
if you have any extension or CTI route points that begin with 9. If you
have the latter and must use a 9 for them the fix is to put the extension or
CTI route point into a partition that is not reachable by any of your
devices and then create a CSS that contains that partition. Then create a
translation pattern that matches the digits of the extension or route point.
Make sure to check the provide dialtone box. Then make the Partition one
that can be reached by your phones and make the CSS the new one which
contains the CSS of the extension or CTI route point. Now you will get 2nd
dialtone after pressing 9 but will also be able to dial the CTI route point
or extension. Remember though that translation patterns ALWAYS have the
urgent routing flag checked so you can't have an overlapping dialplan with a
translation pattern. This above trick is required when deploying Cisco
Emergency Responder for example as the CTI route point is set as 911. Good

Make sure you don't have another DN or route pattern beginning with 9 that
does not offer secondary dial tone.

What is the best way to troubleshoot Outside Dial Tone on a Call Manager
4.1(3)

I am supposed to get the offnet dial Tone right after I press Digit 9 on IP
Phone instead the Offnet Dial Tone appears after pressing 9 and then 1.
