I'd check physical layer. It is possible for T-1s to work amazingly well
with one wire of one of the pairs open. Well enough to just give sporadic
errors. Are the demarcatrion points extrended?

Well, you can put them in loopback, etc.

I'd eliminate the above, never needed it. Let the WIC set LBO automatically.

It is entirely possible that the carrier is clocking the circuit in
which case you don't want the above. Does it work at all without the
clock source internal command? What is telco seeing? Slips?
