Backup and demand-circuit operate completely differently.
The OSPF demand-circuit command simply suppresses hellos, so there is no
traffic as long as there is no topology change. The neighbors will exchange
updates any time there is a topology change.
The backup command causes the backup interface to be shutdown as long as
the primary interface is up. If the primary goes down, it just does the
equivalent of a "no shut" on the interface. The backup interface then
functions normally (the same as it would with no backup command), until the
primary comes back up.
As far as what you need, it depends on the circumstances and requirements.
