This is a subject that has a lot of people confused.
Here is the answer:

1. After you pass your CCIE Qualification (written) exam, you have 18
months to take your lab exam.
If you don't take your lab exam within this time frame, you have to
retake your Qualification exam in order to be eligible once again for
the lab exam.

2. If you take your lab exam within the 18 month period, and if you
don't pass the exam, then you have an additional year to take the lab
exam.
Every time you take the lab exam and you don't pass, you have another
year to retake the lab exam.
However, if it has been 3 years since you passed your written exam and
you still have not passed your lab exam, then you need to retake the
written exam again.
