Now if only it could perform role-based access control via both TACACS+ and
RADIUS it would be a hit ;) For instance, it would be very useful to be able
to say User X can have one priv level on node A via TACACS+, a different
priv level on node B via TACACS+, one level on node C via RADIUS, a
different level on node D via RADIUS. I believe ACS supports RBAC via
TACACS+ but not via RADIUS.

Cisco ACS works great.

It authenticates TACACS and RADIUS requests simultaneously.

Last time I checked it was around $3K for the software and SmartNet is
$1K per year.

It isn't cheap but it works well.

Yes Cisco has it. It's called Cisco Secure ACS.
