Is 4500M has PCMCIA flash card?
I had encounter PCMCIA flash card error.
Every time after uploading a IOS, I always use verify to check the IOS, but I always got error checksum response.
After replacing a new PCMCIA flash card, this problem was solved !!!
So,be caution after you finished upgrading IOS by using the command 'verify' before you reset your router...or you should be on-site.

I have tried hundred times and changed different IOS, but except using original IOS (11.3).Router can't run properly. Whether will I upgrade bootflash first,then try to upgrade flash again.
