4000M Flash issues

Hello,

Does the 4000M support 16 MB flash? in a single chip?
Do I need a bootrom upgrade? Where do I check hardward required bootroms
and versions?
Is the maximum the 4000M memory can hold 2 4MB flash SIMMs?

The router will boot up in Rxboot with single blank 16 MB Flash SIMM,
but will not see flash.:

 I keep getting the following when I do any configuring:

Address Error, address: 0xf1c at 0x103d05e (PC)
Address Error, address: 0xf1c at 0x103d114 (PC)
Address Error, address: 0xf10 at 0x103d510 (PC)
(router is stable when it just boots up and is in Rxboot)

Or when I can get a 2.3 MB IOS (c4000-i-mz.113-11c.bin) image loaded:
