IP phones have a passive filter for auto detect. It lets one
frequency through and blocks the other. I would suspect that
something may be interfering with this process. Make sure the
AC power is clean or change the circuit it is on. There could
be a ground lop between the chassis and the in-line power supply.

Also, make sure your cable is a full 4-pair cable. Some Cisco
gear power the phones on the ethernet pairs and some use the
spare pairs. Weird but true.
