I ran into a similar problem with a 3640 and a 2FE2W.
I finally found that the 12.2.1 LD release ( IP PLUS 3DES for me ) worked.
It does some special diagnostics on bootup ( I have never seen them before )
to query the card.
I tried everything from the newest 12.2 release to some 12.0 releases and
this was the only one that I found worked. Im sure there are others, but I
was tired of:

Copy tftp flash:1:

I'm aware of the IOS issues and issues with 2600 after searching on CCO but
this isn't my real problem since I'm running it on a 3660 with 12.xT
