Try using the T release, it should have it as I have implemented theses configs
 on 12.1(5)T which is now deferred, so try maybe 12.1(5)T9 or later.

I was playing around with ccbootcamp lab 18, when I realize there is no
command under frame-relay map-class to define the policy-map:

2621(config)#map-class frame-relay R14
2621(config-map-class)#?
Static maps class configuration commands:
  default Set a command to its defaults
  exit-class Exit from static map class configuration mode
  frame-relay Configure Map parameters
  help Description of the interactive help system
  no Negate a command or set its defaults

It should have the "service-policy output <name>", right?

Other thing, I can't find any command to use weight fair-queue in
policy-map configuration:

2621(config)#policy-map TEST
2621(config-pmap)# class VOIP
2621(config-pmap-c)#?
QoS policy-map class configuration commands:
  bandwidth Bandwidth
  default Set a command to its defaults
  exit Exit from QoS class action configuration mode
  no Negate a command or set its defaults
  priority Strict Scheduling Priority for this Class
  queue-limit Queue Max Threshold for Tail Drop
  random-detect Enable Random Early Detection as drop policy
  <cr>

I use c2600-jo3s56i-mz.121-7.bin for IOS.
Any comments?
