Roger,

You might want to get the call set up properly before adding on a bunch
of stuff that comes after that.

A useful debug for call set-up problems is also 'debug isdn q921.'

Is the isdn switch-type correct? A signaling protocol error might be
indicative of a mismatch...

Are SPIDs required by your provider (or simulator)?
