Sounds like the exact same problem I had long time ago. I was using Cisco's
TFTP software version 1.0 After the initial check by the router that the
image is on TFTP and after erasing flash the copy wouldn't start.
It was a bug in TFTP software. Solution?
- find newer version of TFTP
or
- after the initial check by the router if the image is on TFTP, close TFTP
program and run it again, then continue with erasing flash and the rest of
the upgrade process.
