Are you running CHAP? Authentication is required for the routers to
exchange names to tie into the dialer map. If there is no authentication,
then the dialer mapping is based upon Caller ID, which of course requires
the telephone numbers in the map to match the incoming caller ID from the
opposite end of the link. Unfortunately, even if the telephone number is
included in the dialer map, the received caller ID may be different than
what is required to dial the other end.

CHAP removes all the ambiguity. Just remember that dialer map names and
router hostnames are a case sensitive match. <painful lesson learned>
