I would change to:
mls qos queue-set output 2 threshold 1 40 100 100 100
as COS 4 maps to queue 2 threshold 1
i'd also trust COS on any ingress trunk ports for uplinks that you may or
may not have!
I'm guessing you've skipped the L2 QoS section
