Typical Linux systems use something like this as an environment variable
PROMPT_COMMAND='printf "\033]0;%s@%s:%s\007" "${USER}" "${HOSTNAME%%.*}" "${PWD/#$HOME/~}"'
Your client program will interpret that and change the title bar.
If you're telnetting to a console port, I don't how you can do that - it's not like you're doing a full login session. Just set the title bar as has been suggested, and save your sessions.
