Hi Olaniyi,
Most likely its the issue with PT/CSS issue - database having hickups!!
Restart the CCM + RIS + CTI services during non-working hours.
Also make sure that the phone firmware you are using is in the compatible
table of Cisco CUCM 6.1.3.
