By far, the two most common bugs are these:
12.4(15)T5-T7 - CSCso87127. This happens on the gateway for inbound
calls to IP AA and the likes.
8.4.1 and 8.4.1 SR1 on the IP phones: CSCsv04751
If it's for outgoing calls to the PSTN and you have an IP phone my
money is on the 2nd bug there. I actually filed this one :)
