FWIW I've had plenty of issues remotely upgrading switches in the field and although it's not something I can clearly put my finger on for why it fails, you do have other options. I do most of my code upgrades through a webserver, if that fails, I move to ftp, if that fails, move to tftp. I've had all of these fail, in particular on 2960's and 3560's. Another option that I tend to go for last, but has proven to be quite reliable is SCP to a *nix box.
