David,
I was not aware you were using MGCP but thought you were using H.323. You should be able to accomplish this with a calling party transformation which should be applied to the gateway. I would recommend that you reset the gateway after you assign the transformation pattern a I have seen bugs where this configuration does not take place until the gateway is reset.
Your other option is to use an H.323 gateway configuration which will utilize the translations on the router.
