For the graded side:

InternetworkExpert seems to hold the uncontested lead.

It's been while since I took their graded exams now. So I'm out of date.
They just came up with a new awesome testing engine - graded labs. I've
been busy or I would have had time to comment by now.

IPExpert is offering at least one free graded lab I've heard. Right now.
Try before you buy.
