Have to confess that I didn't finish it (yet), but "Cisco IP Telephony -
Planning, Design, Implementation, Operation, and Optimization" seemed like a
reasonably good starting point. The appendices are questionnaires and
checklists that likely at least resemble what the partners are using?

they have templates/questionnaire for everything related to voip.
