IPV6 site local address conversion

I am little confused with site-local address for IPv6.

IP : 15.15.25.5

Create IPv6 site-local address /64 with third and fourth octet from IPv4
address.
Then it should be

FEC0:0:0:19::5/64

Is this correct?
