I have 4 questions:

In this scenario, is there a difference between these two commands?

The "device cisco-phone" looks for the presence of a phone in cdp before the
cos is trusted... a much better choice IMHO

Are both enabling trust of the phones cos value?

Yes, but their's will allow a PC to send COS and have those markings
trusted... a bad idea.

Does the "trust device" require the "trust cos" command to take effect?

No.

Consider if you are also using "switchport priority extend cos #" command,
does either option still work as normal?

Yes. That command specifies the marking that PHONE should send the frames
received from the attached port as a certain cos. IMHO 5 should never be set
here, as voice uses that... unless- You had a CCM server attached to an IP
PHONE? LOL
