It all depends on what you have.
Do you have a static or dynamic IP?
Do you get a public or private IP from your ISP? If it is private, it
is going to be much more difficult. You would need something else
external that your home is connecting to that you can connect to.
Does your home router support dynamic DNS? This can be useful if you
have dynamic IPs and it changes often.
The most hopeful situation is that you have a public dynamic IP, with
a basic home router.
On the home router make sure it supports forwarding of ports. So when
you try to telent or ssh to the public address it sends it to an
internal device. If it does then you should be good to go. forward
the telnet port to your home terminal server or ssh whatever it
supports. If you need you can configure a linux box as an ssh server
or install cygwin on a windows machine.
