Are you talking about Media or Signaling security?
I am assuming that you are after protecting both from external hackers...
You need to use some e.g. PIX/ASA can do that.
But if you are not worried about budget *$$$* go for some hardware IPS
(Cisco Detector/IPS or appliance from F5 or Radware DefencePRO) box. They
have built-in signature of well known all SIP vulnerabilities.
I have used F5 and RADware defencePRO for protecting SIP servers and they
really works very well.
