Is it possible you are using/getting fake sfp's ?

Have you verified them with Cisco as genuine?

It may be time to blame China again...

We have replaced the SFPs on both switches about twice so far. The problem
still persists nevertheless.

Frederick had suggested seeing my interface configs. They are as below:
