My guess is:
Remove the power cord
Push the button on the front by the status leds
Plug in the power and hol the button for about 30 secs
On the console type the command init_flash.
Blow away your config.start and vlan.dat files (or a similarly named file -- I can't remember.)
Type "boot" and you have a factory fresh switch.
If you want to preserve the config then rename the startup confog to something else. After you boot you canthen deal with the password on the renamed file.
