Does it happen right after you connect the console without typing anything? If it happens after you typing some commands then avoid using Ctrl.
That is all I found:
"Typing Ctrl-/ when attached to the console port will cause the switch to reboot. There is no workaround. (CSCdw06454)"

It also happened to me on an old 2924XL 6 years ago, but never on a 4500.
