Cisco 2500 cannot do a Flash RW after booted up. Normally it will do
automatic reload and erase itself after executing the copy tftp flash,
if that fail for some reason, you need to boot to ROM mode and use the
ROM to set the ipaddress tftp address (depend on ROM version etc.)

Or last resort, perform the upgrade using another 2500 router which
does not have this
problem, and replace the problematic 2500 (probably ROM version issue)
flash with
the version you want.
