The issue is that the ip phone actually keeps it previous configuration
in nvram on the phone in case of problems. Since you are not getting
the call manager ip address the phone is reverting back to the previous
saved configuration.
The first issue is to figure out why you are not getting the tftp server
address. Do you have the dhcp option 150 and/or 66 setup properly on
your DHCP scope for the call manager's tftp ip address?
If not then hard code the ip address for the tftp server in the phone
itself. To unlock the configuration of the phone, hit the settings
button on the phone and then hit * # # to unlock the phone
configuration.
If you still have problem then pull the power on the phone or the
ethernet cable if using inline power and then hold the * key down on the
phone and plug the power or ethernet cable back in. Continue to hold
the * key and the phone will do a total FACTORY reset. This will
definitely clear any existing configurations.

Guys ;
I have 7960 phone , i am running TFTP server on the callmanager itself
..the TFTP server is working fine and tested .
the problem is that the phones contact DHCP and retrieve everything
correctly except the IP address of the TFTP server ...for some reason it
keeps previoulsy configured ones .
i am also not able to change the tftp server manually on the phones
because cofig is locked and i need to download new config file to get
rid of the old ones .
the question is , is there any trick on the DHCP of the windows which i
need to do to let these phones get the correct address ...i have
configured both option 66 and 150 but no luck , or is there a way to
test that the sever is really returing those values and how
the phones are running SIP load
