Coming in on the end of this long (and detailed) discussion, I can say
that as a matter of personal experience, I have seen this many, many
times in the field, usually due to the misconfig of an ISDN switch, or
perhaps a dying BR1TE/SL1C. I would not at all consider this an unusual
error. And almost invariably, it is without a doubt a problem with the
provider equipment, and not CPE.
