Have you checked that it is the correct IOS for the platform? you haven't
got an XM image on a non XM router?
I have had this problem and it gives the symptoms you describe. Also try
just putting a base IP image then you can TFTP the full image.

Have you checked what the checksum is supposed to be
on CCO?
Have you tried a different image?
