Well...

Technically, you should if you're going to use it in a production network
and you want the testing and line buildout functionality of the CSU. In
a lab environment or for an ugly hack to replace a dead CSU...

Try Graybar,Digikey, etc. for a DB-15 male hood with an 8-pin modular
jack molded in to the top. Should be a couple of bucks. You can pin it
appropriately. If it doesn't work straight-through, try the back-to-back
configuration.

If your network adapter module has jumpers, set them for balanced.
It probably doesn't, unbalanced is more common on E-1 lines.
