Hi Ben,
1) Make sure you can ping the TFTP server.
2) Make sure the "tar" file is in the TFTP root foolder.
3) Make sure you have enough space in flash for the new IOS.
4) If not enough space in flash, then back up the old IOS on the TFTP server and delete the old IOS from flash.
If you still have issues:
1) Try loading the IOS file only (.bin)
