Try "show flash chips" and make sure that you really have two 8M sticks.
There's a trick whereby you can load one 8 and one 4 to get 12, but IOS
will report 16.

If both sticks are really 8 and identical, swap them and try to load an
image of 8M or less. Sounds like you might just have a bad stick in the
second slot.

It could also just be bad contact. Sometimes removing and reseating will
fix it.
