telnet error message

Hi group,

        I made this happen before but now I can't figure out how it happened.

        when i telnet to a router (say, from router A to router B), it said:
"Connection failed: Reason Unknown"

        I'm trying to make this message come back up again, but I can't figure out
what it was (route-map error? some kind of access-list or IP security
function?)

        Now I've just resorted to using "banner" and setting the aaa login
authentication to an unknown tacacs server. This doesn't seem right.

        If that error message jogs anybody's memory, please let me know.
